Gan HaYeled COVID-19 Policies & Procedures Acknowledgement 

Before completing this form, please read through all of the policies, procedures, and guidelines on the COVID-19 Information page.

Each child must have a completed form prior to September 1, 2021.

If there are significant updates to the Gan's COVID-19 policies and procedures, you will be required to submit a new form for your child.

If you have multiple children enrolled in Gan HaYeled, please complete a separate form for each child.
